Hello I'm Claire and I'm a

Care Leaver

Let me explain what this means....
and why, as my GP you need to know about  patients like me...
Care Leavers are young people who have spent at least 13 weeks living in the care of the Local Authority spanning their 16th Birthday
and are  over  the age of 18
we can get extra help from our Local Authority until we are 25
but  we are often on our own
Care leavers have to act very grown up all of the time
it's often difficult for us to ask for help
we often live on our own 
Many of us have experienced abuse or neglect  before we became Looked After
'My past affects my mental and physical health, and could do for the rest of my life'
I may be OK  right now,  but I might need your help if I am...
pregnant, or worried about my own baby
sad  or struggling with self harming or alcohol
So, what should this all mean to you as my doctor?

and how can you help me to lead a healthy, happy life?

well follow these 5 steps and I'll show you...
Step 1: REGISTRATION
It's important to identify us at registration and code our health records:
CARE LEAVER
Please take time to get to know me

ask me about my past and what I want in my future
Step 2:  Practice Trauma Informed Care
Always think how my past experiences could be affecting me
I may  not be telling you how I really feel.....
'I'm an individual, affected but not defined by my past - help me write my own future'

Avoid using labels and 'professional speak'
be my doctor and encourage continuity of care
Step 3: Safeguarding
we are STILL VULNERABLE,
TAKE TIME TO GET TO KNOW US
domestic abuse
criminal exploitation
substance misuse
homelessness
Step 4: Communication
 If it will benefit my care, let other professionals know I am a Care Leaver and what this means
'Ask me what I want and how you can help me'
Ask my permission to speak to my personal adviser (PA) or Leaving Care worker  
Step 5:  Access
I may need your help accessing health services

You hold my health record, I may come to you to find out about the health of my parents or early life 
By taking these steps
stay healthy
you will help us
and safe
