Remote Supports
Hello, I'd like you to meet Sarah and the team that helped her live on her own and become more independent.
How can I move into my own apartment and still be 
supported?
Sarah always wanted to live
independently but didn't think it was possible.
 I can move into my own apartment.
Sarah met with her DDS Case Manager, Patty. Patty suggested looking into remote supports.
Remote supports would be perfect for Sarah.
Sarah and her Planning / Support Team should now complete the (IRSRFM) Tool to see if remote supports are a good fit for Sarah.

If it’s determined to be a good fit, Sarah and her DDS team will use the RS checklist to work through the next steps.    
Once Sarah and her Team have a good idea of how Remote Support will be used, her DDS Case Manager can request funds to help pay for the service if needed.
Sarah loves living independently on her own.

Provides virtual support from a remote location utilizing the Remote Support system that has been designed for the individual. 
Virtual Support Partner
On demand, in person supports sent to an individual’s location as requested by the individual or in response to an electronic devices indication that support is needed.
